Abstract:Objective To explore the clinical value of hysteroscopic transcervical resection of polyp (TCRP) combined with levonorgestrel-releasing intrauterine system (LNG-IUS) in the treatment of abnormal uterine bleeding (AUB) caused by endometrial polyp (EP). Methods The clinical data of 80 patients with EP complicated by AUB admitted to Taikang County People’s Hospital from June 2022 to June 2025 were selected. According to the different intervention plans, they were divided into the control group (n=40, undergoing TCRP surgery alone) and the combined group (n=40, TCRP+immediate postoperative LNG-IUS placement). The follow-up was conducted until 12 months after the surgery. At the same time, the menstrual flow [pictorial blood loss assessment chart (PBAC) score], hemoglobin (Hb) level, endometrial thickness, polyp recurrence rate and adverse reactions were compared between the two groups. Results At 12 months after operation, the PBAC score in the combined group was significantly lower than that in the control group (P< 0.05), the Hb level in the combined group was significantly higher than that in the control group (P<0.05), the endometrial thickness in the combined group was significantly thinner than that in the control group (P<0.05), and the polyp recurrence rate in the combined group was significantly lower than that in the control group (P<0.05). No LNG-IUS perforation, detachment or severe infection occurred in either group. Conclusion TCRP combined with LNG-IUS can effectively reduce postoperative menstrual flow, improve anemia, inhibit endometrial hyperplasia and reduce recurrence rate in patients with endometrial polyps. The combined intervention of the two has a synergistic effect.
