    目的 分析头颈部 CT 血管造影对缺血性脑卒中的诊断价值。方法 选择南阳市第一人民医院 2020 年 1—12 月收治的 750 例疑似缺血性脑卒中患者,均行颈动脉彩色多普勒超声检查(对照组)和头颈部 CT 血管造影(观察组),通过数字减影血管造影对比结果,分析两组患者的检查数据。结果 在 750 例患者的检查结果中,650 例经数字减影造影被诊断为颈动脉狭窄所致的缺血性脑卒中,占 86.7%(P<0.05);观察组行头颈部 CT 血管造影检测诊断的准确率、特异度和灵敏度均显著优于对照组行颈动脉彩色多普勒超声检测(P<0.05);观察组行头颈部 CT 血管造影扫描颈动脉狭窄的检出率显著高于对照组行颈动脉彩色多普勒超声检查(P<0.05)。结论 头颈部 CT 血管造影在诊断急性缺血性脑卒中方面具有较高的准确率、特异度和灵敏度,其成像特征非常典型。

    Abstract:

    Objective To analyze the diagnostic value of head and neck CT angiography for ischemic stroke. Methods 750 patients with suspected ischemic stroke who were admitted to our hospital from January to December 2020 were examined by carotid ultrasound (control group) and head and neck CT angiography (observation group). The examination data of the two groups were analyzed by comparing the results of digital subtraction angiography. Results Among 750 patients, 650 were diagnosed with ischemic stroke caused by carotid artery stenosis by digital subtraction angiography, accounting for 86.7% (P<0.05); the accuracy, specificity, and sensitivity of head and neck CT angiography in the observation group were significantly better than those in the control group (P<0.05); the detection rate of carotid artery stenosis using head and neck CT angiography in the observation group was significantly higher than that using carotid artery color Doppler ultrasound in the control group (P<0.05). Conclusion CT angiography of head and neck has high accuracy, specificity and sensitivity in the diagnosis of acute ischemic stroke, and its imaging characteristics are very typical.
