    目的 探讨动态心电图诊断阵发性室性心动过速(PPVT)的临床效果。方法 选择 90 例 PPVT 患者,均进行动 态心电图检测,根据波形特征分为 A 组(右室起源,n=48)与 B 组(左室起源,n=42)比较各参数。结果 两组平均心室率、 QTcd、QRS时限、ST-T段改变发生率比较,差异无统计学意义(P>0.05);A组QRS电轴小于B组,差异有统计学意义(P<0.05); A 组出现 ST-T 段改变为 81.25%,B 组为 76.19%,经 Logistic 回归分析结果显示,QRS 电轴是 PPVT 发生的危险因素(OR > 1, P < 0.05)。结论 动态心电图诊断 PPVT 具有较高的价值,QRS 电轴是 PPVT 发生的危险因素,通过测量 QRS 电轴可鉴别 起源心室,利于疾病早期诊疗。

    Abstract:

    Objective To investigate the clinical effect of dynamic electrocardiogram in the diagnosis of paroxysmal ventricular tachycardia (PPVT). Methods 90 patients with PPVT were selected for dynamic ECG detection. According to the waveform characteristics, they were divided into group A (right ventricular origin, n = 48) and group B (left ventricular origin, n = 42). Results There was no significant difference in mean ventricular rate, QTcd, QRS time limit and the incidence of ST-T segment changes between the two groups (P > 0.05); The QRS axis of group A was smaller than that of group B, and the difference was statistically significant (P < 0.05); ST-T segment changes were 81.25% in group A and 76.19% in group B. logistic regression analysis showed that QRS axis was a risk factor for PPVT (or > 1, P < 0.05). Conclusion Dynamic electrocardiogram has high value in the diagnosis of PPVT. QRS axis is a risk factor for PPVT. Measuring QRS axis can identify the origin of ventricle, which is conducive to the early diagnosis and treatment of the disease.
